Apparently, the family of the fan saved up money so they can send him to root for the Dodgers in SF. During the game, the family was talking to the third base coach about him wanting to meet Kemp, and he said that he’d see what he could do.

Matt the two-time All-Star met a disabled fan and his family…

and proceeded to give him an autographed baseball. Then almost everything else he was wearing

A mere gesture for Kemp, and a lifelong memory for this fan and his family.
